This online form is for staff in the Office of Undergraduate Education (OUE) to provide feedback and input directly to Steven Girardot, Vice Provost for Undergraduate Education.  Examples may include: nominating colleagues for  “shout outs” or other professional or personal recognition (which may include an important life event or accomplishment); making us aware of any concerns; providing suggestions for improvements, enhancements, or ideas for new initiatives; and sharing any other comments related to your work in OUE and how we can better support you.  

Steven will review submitted feedback on a regular basis. Your submission will be anonymous unless you choose to identify yourself. Please note: this form should not be used to report time-sensitive matters or to replace other communication mechanisms or appropriate GT OHR processes/policies.
