Sexual Violence Confidential Referral Form

The purpose of this form is to refer students who have made a disclosure of sexual violence to a VOICE Advocate for follow-up.
Responsible employees should also follow policies and procedures related to reporting to the Title IX Coordinator.
 
VOICE Advocates provide supportive services to primary and secondary survivors of sexual violence including crisis intervention, safety planning, information about reporting options, referrals and accompaniments to services, academic and housing accommodations, and emotional support.

Responding to Reports of Sexual Violence Checklist

Before submitting a referral to a VOICE Advocate, please complete the checklist below to ensure the victim/survivor's immediate needs are met.
Did you determine if the victim/survivor is in immediate danger or in need of emergency medical care?
Did you contact a VOICE Advocate for immediate support for the person reporting the incident?
Did you share information about resources to the person reporting the incident?
Did you report this incident to the Title IX Coordinator?
